Procedures

If your Myna Health Insurance Card (MYNA HOKENSHOU) or Health Insurance Eligibility Certificate is lost or stolen, report it to the police immediately.
If you lose your Health Insurance Eligibility Certificate, you need t to contanct and complete procedure with the Health Insurance Society.

Lost Myna Health Insurance Card

If you lose your Individual Number Card, call the My Number General Toll-Free Number immediately to temporarily suspend its functions and prevent misuse.

If your card is lost or stolen outside your home, report it to the nearest police station.

Reissue of an Individual Number Card

To have your Individual Number Card reissued, contact your local municipality.
After receiving your new card, log in to the Myna Portal to confirm that your health insurance card is registered. If it is not, register it again.

If you receive medical treatment while your Individual Number Card is being reissued

If you need medical treatment while your Individual Number Card is being reissued, you must obtain a Health Insurance Eligibility Certificate. Apply to your health insurance society for this certificate.

Change of Individual Number

If your Individual Number is leaked due to theft of your Individual Number Card or for any other reason and may be used fraudulently, you can request to have your Individual Number changed. This change may also be initiated by the mayor of your municipality. For details, contact your local municipal office. If your Individual Number is changed, you must notify your health insurance society.

If you do not have a Myna Health Insurance Card and would like to continue to use your Health Insurance Eligibility Certificate to receive medical treatment, please apply for a reissuance of your Health Insurance Eligibility Certificate.
