Change of Name and Address

When you change your name, you must complete procedures with both your employer and the Health Insurance Society.
To check how to notify your employer, please search for “Name Change” or “Change of Address” on SharePoint.

Procedures Related to Change of Name and Address

Name change Please submit the following required documents without delay.
(Refer to the “Application Forms” page for information on where to submit application forms.)

  • Application for Name Change
    PDF Completed Sample
  • Family Register (Not required for marriage or divorce)
  • Valid Eligibility Confirmation Document issued under your previous name (only if issued)
* Even if you plan to continue using your maiden name at work, you will need to go through the process of changing your name.
Address change Notify your employer only.
(Voluntary and continuous insured persons must submit a change form to the EY Health Insurance Society.)
Cross out the old address on the back of your Eligibility Confirmation Document and write the new one.
If you registered your address on the EY Health Insurance Society website, update it yourself.
