High-cost Medical Care Expenses

The EY Japan Health Insurance Society automatically calculates and pays the total amount of high-cost medical expenses and additional benefits for any out-of-pocket medical care costs exceeding the ceiling amount at a later date based on the statement of medical expenses (insurance claim statement) sent from the healthcare provider.
(This excludes amenity bed costs and other expenses not covered by insurance and the cost of inpatient meals.)

High-cost Medical Care Expenses

High-cost Medical Care Expenses and Co-payment Reductions (Additional Family Medical Expense Benefit)

The EY Japan Health Insurance Society reimburses insured persons and dependents for high-cost medical care expenses paid at the billing counter for inpatient or other medical care at a later date. A co-payment reduction (additional family medical expense benefit) is also offered as a unique benefit from the EY Japan Health Insurance Society.

Insured persons and dependents can limit payments at the billing counter of healthcare providers to the ceiling amount of out-of-pocket costs by applying for and presenting an Eligibility Certificate for Ceiling-Amount Application. These payments are calculated automatically based on the "medical fee statement" sent from the medical institution to the Health Insurance Society (the insured person or dependent does not need to submit an application form). ) Payment is made approximately three to four months after the month of treatment.

*Insured persons and dependents can reduce the burden of co-payments when the total amount of out-of-pocket payments for medical insurance or long-term care exceeds a certain amount through programs such as the high-cost medical care expense and high-cost long-term care cooperation system for reimbursements of excess medical care costs or certificates issued for specified disease treatments to reduce the ceiling amount for specified diseases incurring high-cost medical care expenses.

*1 If a household has qualified for High-Cost Medical Expense benefits three or more times within the past 12 months, the maximum out-of-pocket limit will be reduced from the fourth time onward.
*2 If the total out-of-pocket expenses during the calculation period (from August 1 of the previous year to July 31) exceed the annual limit for the income category as of the reference date (July 31), the excess amount will be paid as a benefit.
*3 For those who are confirmed to have a standard monthly remuneration of 150,000 yen or less as of July 31, 2027, an annual maximum of 410,000 yen will be applied, and any amount exceeding the annual maximum incurred from August 2027 onward will be reimbursed on a retrospective payment basis.
*4 Insured persons and their dependents who are exempt from municipal inhabitant tax, or insured persons and their dependents who do not require public assistance (welfare) by being classified under the "low-income earner" category. Please note that individuals with a monthly standard remuneration of 530,000 yen or more do not qualify as "low-income earners" even if they are exempt from municipal inhabitant tax, and the standard classification based on monthly standard remuneration will apply.

*1 Figures in square brackets represent the ceiling amount from the fourth month. The maximum amount decreases from the fourth month of high-cost medical expenses if medical care costs for the same household in the last 12 months satisfies the payment requirements for high-cost medical care expenses for three or more months.
*2 Income earners at the same level as active employees are insured persons or dependents who have a standard monthly remuneration of 280,000 yen or more.
However, these insured persons and dependents may pay the standard 20% out-of-pocket costs by filing an application with the Demo Health Insurance Society if:
  • The individual is age 70 or older and earns less than 3.83 million yen annually without any dependents
  • The individual has a dependent or former dependent age 70 or older and has a total combined annual income of less than 5.20 million yen
*3 When the total out-of-pocket expenses incurred over one fiscal year (August 1 of the previous year through July 31) exceed the "Annual Maximum" for the income bracket as of the standard date (July 31), in principle, the amount exceeding the ceiling will be paid as "High-Cost Medical Care Expenses."
*4 If, as of the reference date (July 31), you are classified as having a standard income level, and the total amount of your out-of-pocket expenses for outpatient medical care during the months in the calculation period (August 1 of the previous year through July 31) in which you were classified as having a standard income level exceeds JPY 216,000, the amount exceeding JPY 216,000 will be reimbursed. Similarly, if, as of the standard date (July 31), you are classified as a low-income individual, and the total amount of your out-of-pocket expenses for outpatient medical care during the months in the calculation period (August 1 of the previous year through July 31) in which you were classified as low-income exceeds JPY 96,000, the amount exceeding JPY 96,000 will be reimbursed. If you have been a member of our Health Insurance Society throughout the calculation period, the payment will be made automatically and no application is required. If you changed insurers during the calculation period, you will need to apply for the payment, so please contact the Health Insurance Society.
*5 For those who are confirmed to have a standard monthly remuneration of 150,000 yen or less as of July 31, 2027, an annual maximum of 410,000 yen will be applied, and any amount exceeding the annual maximum incurred from August 2027 onward will be reimbursed on a retrospective payment basis.

Persons Who Are Receiving Benefits from a Medical Care Costs Subsidy System for Infants and Children

If subsidies are available from public institutions such as the national government or local governments (prefectures and municipalities), subsidies will be given priority. In order to prevent double benefits for local government subsidies and health insurance benefits, we do not, in principle, provide automatic family medical care cost supplement benefits to dependents who are covered by the "infant," "child," or "high school student" medical expense subsidy system in area of residence. If you are unable to receive medical expense subsidies from the local government due to income restrictions or other reasons, please contact the Health Insurance Association.

Eligibility Certificate for Ceiling-Amount Application

Insured persons and dependents can apply to have a certificate issued by the EY Japan Health Insurance Society in advance for any out-of-pocket payments exceeding the ceiling amount to present at the billing counter of the healthcare provider in order to limit payment to only the medical costs up to the maximum amount. After you complete the procedure below, an Eligibility Certificate Card will be sent to your specified address.
If you use your Individual Number Card (My Number Card) as your health insurance card, you will be exempt from paying any out-of-pocket expenses that exceed the maximum amount under the High-Cost Medical Expenses System without undertaking any prior procedures.
There is no need to apply for an Eligibility Certificate for Ceiling-Amount Application in advance.

* To use your Individual Number Card as a health insurance card, you must register on the "Myna Portal site", etc.

* Medical institutions that do not support the use of the Individual Number Card as your health insurance card are not exempt.
In addition, if the medical institution cannot confirm your eligibility using your Individual Number Card, you may not be able to receive an exemption.
Once you have finished using it, please be sure to return it to the Health Insurance Society.
(Note) If you are classified as a "low-income earner", you must apply for "Eligibility Certificate for Ceiling-Amount Application and Reduction of the Standard Amount of Patient Liability(限度額適用・標準負担額減額認定証)".

* If you present your Myna Health Insurance Card at the time of treatment, you do not need to apply for an Eligibility Certificate for Ceiling-Amount Application.
However, if you fall into the low-income earner category, you are required to apply in advance for the Eligibility Certificate for Ceiling-Amount Application and Reduction of the Standard Amount of Patient Liability, even when using your Myna Health Insurance Card.

Regardless of whether or not you submit the Eligibility Certificate for Ceiling-Amount Application, the amount calculated by deducting 20,000 yen from the medical care costs paid at the hospital billing counter will be paid approximately 3 months after the month in which medical treatment was received (If the calculation result is less than 500 yen, no payment will be made).
*High-cost medical care expense benefits are calculated automatically and do not require an application.
In either case, the amount of final co-payment is the same.

Four Months or More of High-cost Medical Care Expenses (Multiple Occurrences)

The co-payments from the fourth month in which high-cost medical care expenses exceed the ceiling amount vary as follows when insured persons and dependent have paid high-cost medical care expenses three or more times in one year (last 12 months) in the same household.

Regarding "the Annual Maximum"

Effective August 2026, an "Annual Maximum" has been established to help those requiring long-term medical treatment better plan for their "future medical expenses," which have been a source of anxiety.

When the total out-of-pocket expenses incurred over one fiscal year (August 1 of the previous year through July 31) exceed the "Annual Maximum" for the income bracket as of the standard date (July 31), in principle, the amount exceeding the ceiling will be paid as "High-Cost Medical Care Expenses."

* The "Annual Maximum" is calculated on a household basis, and the ceiling amount varies depending on the age and income of the insured person.

* If additional benefits are received, the additional benefit amount is deducted from out-of-pocket expenses, and the remaining amounts are totaled.

* If the insurer changes during the calculation period, the "Annual Maximum" applies separately to the medical care received during the period of enrollment with each insurer.

Special Exception for Specified Diseases

Patients with hemophilia, acquired immune deficiency syndrome (AIDS) administered antiviral agents, and chronic renal failure requiring dialysis for an extended period only need to pay 10,000 yen per month to a healthcare provider once certified as a person with a specified disease. However, patients who require dialysis have a co-payment of 20,000 yen if their standard monthly remuneration is 530,000 yen or more. (This applies to both insured persons and dependents.)

Procedures

Individuals undergoing treatment for specified diseases attach a physician’s certificate to the Application for Issuance of Certificate of Medical Treatment for Specified Diseases to the EY Japan Health Insurance Society to receive a Certificate of Medical Treatment for Specified Diseases. The insured person or dependent presents the Certificate of Medical Treatment for Specified Diseases issued by the EY Japan Health Insurance Society with their Myna Health Insurance Card or Eligibility Confirmation Document to the billing counter of the healthcare provider.

* Please submit the application promptly when undergoing treatment for a specified disease because only applications received by the EY Japan Health Insurance Society by the end of the month are applied to the current month.
